（2）对所有来自或途经国内疫情高风险地区的来沪返沪人员，一律赋“红码”，并实施14天集中隔离健康观察，实行相应频次的新冠病毒核酸检测；隔离期满且核酸检测均为阴性，转赋“绿码”。
（3）对所有来自或途经国内疫情中风险地区的来沪返沪人员，一律赋“黄码”，并实施14天严格的社区健康管理，实行相应频次的新冠病毒核酸检测。社区健康管理期满且核酸检测均为阴性，转赋“绿码”。
3.入境目的地为本市的人员，在解除14天集中隔离后，如在本市有固定居住地且满足居家健康监测条件（“一人一户”或“一家一户”）的，落实7天居家健康监测。其间，居家健康监测人员及同住人“非必要不外出”。因生病就医等特殊情况确需外出的，安排专用车辆闭环转送。
4.入境目的地为本市、但无固定居住地或不满足居家健康监测条件，或入境目的地为外省市但需在沪停留的人员，在解除14天集中隔离后，入住指定健康监测酒店落实7天健康监测，“一人一间（套）”或“一家一间（套）”。其间原则上不得离开房间，由酒店提供餐食、外卖、包裹等转送服务。因生病就医等特殊情况确需外出的，安排专用车辆闭环转送。
5.入境人员解除14天集中隔离健康观察时，由专用车辆闭环转送至居家健康监测地或指定健康监测酒店。严格落实每日早晚2次体温监测和个人健康状况监测。在健康监测的第2天、第7天，由专用车辆闭环转送至指定医疗机构开展核酸检测，或由务人员上门采样。相关人员如健康监测期满，且核酸检测结果为阴性、无新冠肺炎相关临床症状，由所在社区或指定酒店解除健康监测。
6.健康监测期间，入境人员及同住人如未履行相应义务或违反规定，造成疫情传播风险的，将依法依规采取惩戒措施，严肃追责。
